Job Description Job Description Job purpose The Transitional Care Coordinator is responsible for educating patients about our Home Visit program and scheduling appointments to avoid re-hospitalizations. This position cold-calls patients, therefore having a sales-mindset is ideal! This is an in-person position, located at our M&H Housecalls office in Cape Coral FL. Duties and responsibilities Outbound calls to patients to introduce our Home Visit Program Scheduling patients by geographical area Using proprietary computer database to enter and assign patients to providers Submitting paperwork to Primary Care Physicians Making follow-up calls to patients post-appointment(s) to identify further needs Communication to referral sources in the community Qualifications Minimum 1 year of experience scheduling patient appointments Experience with sales a plus! Minimum 1 year of experience in a healthcare setting High School Diploma or GED Self-motivated in remote setting; ability to be productive, free of distractions, in a 'home-office' environment (orientations, trainings, meetings may require travel from time to time) Utilization of many computer software systems, our platform, hospital systems, Microsoft etc. Ability to multi-task Bi-Lingual (English & Spanish) preferred not required Excellent written communication skills Must be comfortable being on the phone for several hours a day Must be friendly & courteous and have patience on the phone, as we are very often assisting an elderly demographic Compensation negotiable based on experience
